|Open Enrollment Period||Open Enrollment is a period of time which eligible individuals can elect to enroll in, or transfer between, Qualified Health Plans.|
|Special Enrollment Period||Special Enrollment Period (SEP) is a time outside of the Open Enrollment Period during which you and your family may qualify to sign up for health coverage. |Divorce or Death or enrolled in Medicare||Loss of employer or individual coverage because of divorce from policyholder, death of policyholder or policyholder enrolled in Medicare.|
|Loss of Medicaid or CHIP coverage||Loss of Medicaid or CHIP coverage.|
|New dependent through marriage||Addition of new dependent as a result of marriage.|
|Birth, adoption, placement for adoption or foster child||Addition of new dependent as a result of birth, adoption, placement for adoption, or placement of a foster child.|

Tobacco user includes any person who has regularly used tobacco products (cigarettes, pipe, cigar, snuff, or chewing tobacco) within the last 6 months. Regular use means an average of four or more times per week, excluding tobacco use for religious or ceremonial purpose.
Note: Tobacco use does not apply to applicants under the age of 18.
